Consent for Outpatient Mental-Health Treatment

Nature and purpose of services

Global Therapy, Inc. provides outpatient behavioral-health assessment, diagnosis when clinically appropriate, treatment planning, psychotherapy, psychoeducation, care coordination, and related services. Services are provided by independently licensed or supervised clinicians acting within their lawful scopes of practice. Your clinician will identify their credentials and discuss the treatment methods reasonably expected to be used.

Therapy is voluntary unless it is required by a court or another lawful authority. You may ask questions, decline a proposed intervention, request a referral or second opinion, or end treatment. Ending treatment does not eliminate responsibility for services already provided.

Possible benefits, risks, and alternatives

Therapy may improve coping, functioning, relationships, insight, emotional regulation, and relief from symptoms. It can also involve discussing painful experiences and may temporarily increase sadness, anxiety, anger, grief, sleep disturbance, conflict, or other discomfort. Changes made during therapy may affect relationships, work, housing, school, substance use, or other areas of life. Results cannot be promised.

Alternatives may include no treatment, another clinician or treatment approach, group or family services, medical or psychiatric evaluation, medication management, a higher level of care, community support, or other services. Your clinician will discuss reasonable alternatives and referrals when clinically indicated.

Treatment planning and participation

Your clinician will work with you to identify concerns, goals, and an appropriate plan. Treatment generally requires honest participation, attendance, feedback, and agreed work outside sessions. The plan may change as your needs become clearer. You may request an explanation of your diagnosis, treatment approach, expected course, alternatives, or fees at any time.

Confidentiality and its limits

Global Therapy protects your health information under applicable federal and state law. Information may be used or disclosed for treatment, payment, and health-care operations as permitted by law. Other disclosures generally require written authorization unless a legal exception applies. The Notice of Privacy Practices provides additional information.

Depending on the law and circumstances, confidentiality may be limited when:

  • There is suspected abuse or neglect of a child, or abuse, neglect, or exploitation of an adult protected by reporting law.
  • Disclosure is legally permitted or required to address a serious and imminent threat to the health or safety of a person or the public.
  • A valid court order, subpoena, warrant, or other legal process requires a response. A subpoena does not automatically permit immediate disclosure; Global Therapy will evaluate privilege, authorization, notice, court-order, and other requirements.
  • A health-oversight agency, licensing board, insurer, or government program lawfully requires information.
  • Disclosure is otherwise required or permitted by applicable law, including certain emergency, workers' compensation, or law-enforcement circumstances.

Clinicians may obtain professional consultation or supervision as permitted by law and professional standards. Reasonable efforts will be made to limit identifying information unless identification is necessary for treatment, supervision, safety, payment, or another lawful purpose.

Emergencies and communication

Global Therapy is an outpatient practice and does not provide continuous crisis coverage. Portal messages, voicemail, email, and text are not continuously monitored and must not be used for emergencies. If you may act on thoughts of suicide or violence, cannot remain safe, or have another emergency, call 911, call or text 988, or go to the nearest emergency department. Ordinary voicemail, text, and email may not be completely secure and should be used primarily for scheduling, billing, and other administrative matters rather than detailed clinical information.

AI-assisted documentation and temporary recording

Global Therapy clinicians may use Blueprint, an AI-assisted documentation system, to create a draft progress note. When used, the system temporarily audio-records the session, processes the recording to generate a draft note, and automatically deletes the recording after the note is generated. The clinician remains responsible for reviewing and completing the clinical record. The recording will not be used for marketing, research, or training without separate authorization.

Use of Blueprint is optional. You may decline or withdraw consent at any time by telling your clinician or contacting Global Therapy. If you decline, your clinician will document sessions without Blueprint, your decision will be recorded in your clinical record, and your ability to receive therapy will not be affected.

Clinical services are not forensic services

Routine therapy is not a custody evaluation, disability evaluation, fitness-for-duty evaluation, expert assessment, or other forensic service. Global Therapy cannot promise that a clinician will prepare a forensic opinion, report, deposition, or testimony. A subpoena or court order will be handled according to law. When court-related professional services are accepted and provided, the financial policy applies.

Ending treatment

You may end treatment at any time. Global Therapy may recommend transfer, pause services, or end treatment when services are no longer clinically appropriate or effective; your needs exceed the practice's scope; attendance, payment, safety, or boundary concerns interfere with treatment; a clinician becomes unavailable; or another lawful and ethical reason applies. When reasonably possible, the clinician will discuss the decision, provide appropriate notice, offer transition planning, and provide referral resources.
